چکیده

conclusions full echocardiographic examination of the right ventricle, including rvot is essential for the diagnosis of this disease. surgical correction appears to be a successful strategy for management of this rare condition in adults. case presentation we report a 40-year-old man who was referred to cardiology clinic for work-up of dyspnea and a heart murmur found on his physical examination. a transesophageal echocardiogram (tee) revealed a muscular subpulmonic stenosis and a small perimembranous ventricular septal defect (vsd) with left-to-right shunt and severe right ventricular outflow tract obstruction (rvot). right heart catheterization showed an estimated gradient of 75 mmhg across the two chambers due to subpulmonary stenosis. the pressure in the proximal right ventricular chamber was 100/25 mmhg and 25/17 mmhg in the distal chamber. surgical correction was successfully performed. a postoperative tee showed no residual gradients across the rvot. introduction muscular subpulmonic stenosis causing right ventricular outflow tract obstruction is an uncommon cardiac disease, especially when first diagnosed in adults. diagnosis can be challenging when it is minimally symptomatic.

Mesenteric Fibromatosis Causing Ureteral Stenosis

Mesenteric fibromatosis is a rare benign disease characterized by proliferating fibrous tissue in the bowel mesentery. We report a case of aggressive mesenteric fibromatosis with ureteral stenosis arising in a 46-year-old woman who suffered from intermittent right abdominal pain. Computed tomography revealed a right retroperitoneal mass with right ureteral stenosis at the level of the right com...

Tophaceous gout causing lumbar stenosis

RATIONALE Gout in the spine is very rare. The clinical symptoms of the spinal gout are various and lack of specificity. The authors report a case of spinal gout causing lumbar stenosis. We never find such wide-invasive spinal gouty lesion in the published studies. PATIENT CONCERNS A 68-year-old male had low back pain radiating to bilateral lower limbs, accompanying with intermittent claudicat...

Discrete subpulmonic membrane in association with isolated severe pulmonary valvar stenosis

BACKGROUND Subpulmonic membrane as a cause of right ventricular outflow tract obstruction in patients with concordant ventriculoarterial connection and intact ventricular septum is considered to be rare. CASE PRESENTATION A 7-year-old boy was referred to a tertiary care hospital with complaints of dyspnea on moderate exertion and palpitations of about 2 years duration. Physical examination re...

Giant multiloculated left ventricular outflow tract pseudoaneurysm causing severe extrinsic compression of subpulmonic infundibulum.

A 14-year-old female presented to us with a history of palpitation and progressive dyspnoea for the last 2 months. She also had a history of prolonged fever 4-months back. On examination, she had a pulse rate of 110 bpm and a blood pressure of 110/60 mm Hg. Her precordial examination revealed a hyperdynamic and downwards displaced cardiac apex beat. The cardiac auscultation revealed a pansystol...

Anomalous Papillary Muscle Insertion Causing Dynamic Left Ventricular Outflow Tract Obstruction without Hypertrophic Obstructive Cardiomyopathy

Anomalous papillary muscle insertion directly into the surface of the mitral valve leaflet is rare, especially in a subject without apparent evidence of hypertrophic cardiomyopathy. We present a case of this isolated congenital malformation producing two hemodynamic sequelae of dynamic left ventricular outflow tract obstruction and severe mitral regurgitation.

Right Coronary Cusp Prolapse Resembling Subpulmonic Stenosis in an Old Adult Patient with Ventricular Septal Defect

Ventricular septal defect (VSD) can be associated with various complications such as aortic regurgitation (AR). AR in VSD come from a deficiency or hypoplasia of the conal septum which leads to abnormal apposition in diastole and prolapse of the poorly supported noncoronary or right coronary cusp through the VSD into the right ventricle resembling subpulmonic stenosis and subsequently results i...
